Re: Dental Screening
Dear Parents of Freshman Students and out of state transfer students:
Legislation was passed by the State of Iowa, effective July 1, 2008, requiring all children entering Kindergarten, ninth grade and transferring students from out of state to have a dental screening. The purpose of the dental screening is to improve the oral health of Iowa’s children.
Attached is the required Certificate of Dental Screening form. A screening completed within one year prior to August 4, 2008 is acceptable. So, if your child has recently been to the dentist please take this form back there for him/her to sign. Completed forms should be returned to me at school.
