Re:  Dental Screening

Dear Parents of Freshman Students and out of state transfer students:

Legislation was passed by the State of Iowa, effective July 1, 2008, requiring all children entering Kindergarten, ninth grade and transferring students from out of state to have a dental screening.  The purpose of the dental screening is to improve the oral health of Iowa’s children.

Attached is the required Certificate of Dental Screening form.  Therefore, if your child has recently been to the dentist please take this form back there for him/her to sign.  Completed forms should be returned to me at school.
